Medical Records Procedures

ROI specialist, temporary position working for Office Team, working the ROI desk at Lakeview Hospital, Bountiful Utah, as a HealthPort contracted employee- 3/30/13 to August 23, 2013(temp position)

Duties include performing ROI for faxed requests for continuity of care, providing ROI to customers who walk-into the department, verifying dates of stay to insurance companies, assisting patients over the phone, with the procedure of how they can obtain a copy of their records, and process mailed requests for medical records.  As overlap of working this position I was involved with the other HIM staff members charged with complying with scanning, storing, retrieving, and securing physician compliance of EHR documentation.  This exposure allowed me to get a well-grounded feel for the workflow design and processing of the paper chart as it moves through to EHR…invaluable!

Remote Level 1 Coder, Parallon Business Corporation, HSC- Nashville- April 2012 to September 2013

I worked from home, using business-provided computer and their selected software programs of McKesson/HPF and Medi-Tech, along with 3M-encoder equipment/programs, applying ICD-9-CM codes and CPT & HCPCS codes to reports from physician offices and ancillary departments (lab & radiology) for providers in Utah and Tennessee.

Health Information Management Tech, Ogden Regional Medical Center- January 2010- April 2012 (part-time Friday and Saturday evening)

Employed as file clerk at Ogden Regional as a second job, while maintaining a full-time position as the HIM Director at Christus Health. My file clerk duties included after-hours release of information for continuity of care purposes; personal requests; and internal department requests.  I assisted physicians with chart completion by pulling their charts and placing them in the work area. I maintained department standards of filing loose papers and re-file of charts for three out of ten sections of medical records.  I also accessed the computer system to locate medical records, as well as to log out charts to different areas in the hospital.  I assisted the physicians with locating their charts, and chart completion.

I worked on other projects, as requested by the HIM Director, to include, but not limited to: preparing medical records for purging and off-site storage; worked on a project preparing hospital forms with scanning labels, in preparation for the EHR.

Director of Health Information, CHRISTUS St. Joseph Villa, Salt Lake City.  January 2004 through January 31, 2011.

This position was eliminated by new owner purchase, in 2011.  Prior to job elimination, my duties included employee hiring (and termination), performance review, cross-training, and coding education.  I was solely in charge of managing the medical records department for this 200+ bed facility.  I oversaw the unit clerk ICD-9-CM coding, and provided after-hours coding for new admissions.  I also provided inpatient and outpatient coding for the geropsych unit.   I reviewed outside facility billings submitted to us from outside ancillary provider services to assist our billing office with verification that those services had been performed, and payment should be made to the vendor.  I served as HIPAA Privacy Officer Designee to the facility administrator.  I provided all release of information for the facility.  I served on the QA Committee, Compliance Committee, and chaired the Forms Committee, as well served on the Nurse Leadership Team.  For these various committees I performed audits and provided results. I ensured that the Medical Records Department was in compliance with State, Federal and JCAHO regulations.  As an RHIT, I also carried the credential for the facility, which eliminated the need for an outside consultant.

Transportation Department Director, CHRISTUS St. Joseph Villa- January 2008- January 31, 2011.

This position was added to the aforementioned duties, in 2010, and included the supervision of the transportation department to ensure that residents were transported safely, and timely, to their medical appointments.  I also monitored the customer service practice of my associates. 

Medical Records Director, Senior Living Concepts of Santa Barbara- August 2001 to November 14, 2003.

 

Responsibilities included overseeing the medical records department and the supervision of the medical records designee.  Job duties are as detailed above, in addition to data entry of physician’s orders into the computer to generate the MAR, TAR and RNA reports on a monthly basis.  In March of 2003, I was asked to assume the position of HIPAA Privacy Officer, in addition to my other duties.  I in-serviced the management and other staff on HIPAA, created the necessary first policies and procedures for HIPAA.
